Please try uninstalling and reinstalling using the mini-mapbook release from
the downloads page. If that has no effect, next step is to
- close all arcgis programs (arccatalog, arcmap),
- navigate to your user profile esri directory (type "%appdata%\ESRI" in a
Windows Explorer location bar), usually something like
"C:\Users\username\AppData\Roaming\ESRI"
- search for and rename "Normal.mxt"
- Run Arcmap
- If this didn't help, close Arcmap and rename Normal-backup.mxt to Normal.mxt
Custom toolbar locations etc. are stored in Normal.mxt. Arcmap will create an
new default one if it's not found on startup.
Something else to try is using System Restore (found in Control Panel) to reset
your system to the last time you remember Mapbook working. Please note that
this may mean you will have to reinstall or configure other applications that
may have changed in the meantime.

Unistall was successfull. Now, I cannot add the dll's again. It says "no new
objects were added" when I try to add the dll's from the visual basic folder.
"- search for and rename "Normal.mxt""
rename it to what?

I take that back....
The tab and toolbar are there, but when I try to do anything an error window
(DSMapBookPRJ) pops up. "Could not find index layer in DrawPage routine"

rename to anything other than `Normal.mxt`. It's just so that Arcmap doesn't
find or use it. It's a test to determine whether the problem lies within the
startup template or somewhere else.
I'm afraid I don't have any other troubleshooting ideas for you at this time. :(
