Mapbook error #93

What steps will reproduce the problem?
1.clicking on create/update mapbook
2.
3.

What is the expected output? What do you see instead?
is supposed to open Grid Generator Wizard

Getting this error.
Error Loading the form: 547 - Odject variable or With block variable not set

Attempting to continue the load...

What version of the product are you using? On what operating system?
Windows 7 x64

Please provide any additional information below.
Wizard does open after selecting OK.  I can then enter all information in 
needed fields.  Then I get the following error.

GenerateGrids - 530 - Error in CalculateRowColCounts: Grid width 7900.000000000 
exceeds range specified (0 - 0)

I have attached screen saves of the wizard as I went through the wizard.  I get 
both errors when trying a new grid or using an existing grid.

the only suggestion I have at this time is to reinstall using the minimal 
package from the downloads page and/or going through the various methods 
outlined on InstallOnVista (which also applies to Win7) and InstallOn64bit.
