Add tests for all the different display modes

[ ] display
[ ] display summary
[ ] structure
[ ] structure summary
[ ] structure verbose
[ ] debug
[ ] document this aspect of all elements

Make sure that there is only one render, even when a mode overridden from display to anything on first configured. Currently have to set the default to display in FhirPresentableElement
