As a PHO I can push requests to Self-Declare through notifications. #314

Roadmap: Self-Declare

What: Provide a way for a PHO admin to push requests to individuals or groups to Self declare. For example, where an outbreak is suspected or predicted, send SD requests so that the results can help determine actions to take.
