Create boost request forms #14

Create forms for boost requests that posts to correct forum so can sort out date and time with the customer

Boost content page must be created first. I think the form should have a date picker with wednesdays available. Check boxes for bosses, and upfront price calculator. Items costing on a per item basis cannot be taken into the price calculation.
