@page :first does not work in combined mode

Hi,

I would like to style my first page differently. Unfortunately, the css declaration in a @page :first pseudo element affect every page.

I guess it's an issue related to #36 .

Could you do something about it ?

Unfortunately I don't see a way to fix this easily.., when #35 is implemented, I will expose the page number so per page styling could be done.
