Improved grammar and formatting in `README.md`. #23

There were a few sections which were unreadable for me - a native English speaker:

Consequently, I've corrected them the best that I can. Solely three lines I couldn't correct, which have been commented: https://github.com/zhaouv/vscode-markdown-everywhere/pull/23/files#diff-b335630551682c19a781afebcf4d07bf978fb1f8ac04c6bf87428ed5106870f5R60-R71. However, those should be easily rephrasable my the author.
