Unstable connection establishment in some scenarios

There have been several reports of difficulties establishing connections under 
some circumstances:

- Connection from two peers in a single host, using two different names. First 
connection succeeds while second connection throws an exception;

- Connection between two diameter hosts on the same machine;

- Reconnection by client after successful disconnection from server;
