reorganised for many ports #8

This is a PR requesting review for the reorganisation for porting.

Some functionality has not yet been put in place under this re-organisation, namely AUHAL device scanning and ALSACGO. Wanted to kick-start review, especially w.r.t. {README.md,Porting.md, sio/host, sio} while I get the linux IO back up.

@hajimehoshi I thought that in particular the README.md port statuses with question marks and the default host entry points in host/entry_{runtime.GOOS}.go might be something you'd particularly want to comment on.

Comments addressed, alpha state yelled at top of our lungs, @hajimehoshi can't review in time, the project needs the new face. so merging, we can continue any review in the form of issues w.r.t. the merged code.
