finish protocolizing Shell receptor #111

Doing this as preparatory work for #109, because I want the use case to be having the HTTP receptor route shell commands to the the shell receptor, but for that to work well has to be exposed as protocol, not just as receptor with manually installed expectations.

f5c5f6159687e1e03 adds command line parsing protocol which the shell receptor now expresses, instead of hard-coding that process and expectation
