Allow user to create an account and do an email in one step.

Relayer create/initialize the account based on the accountKey in headers (maybe to address)
Relayer create EmailOp based on the subject of the same email.

Things to consider

Nullifier will be same for both operations as it is derived from the same email. This needs to be handled in the contract.

Would be better to after #43.
