Add a short 'Troubleshooting' section, relevant for the recent bug reports #68

A common issue is the version of the the database, which you can check by its size

Also, the chr{CHRM} is important on the path to the reference panel (if you're using 1KG/EUR).

Actually, maybe this is a bit out-of-date. Just in the last few minutes I finally worked out that there really is a problem with chr22, and how to fix it. I'll send a pull request about it. Basically, we can allow users to replace {CHRM} with 22 if they like, but it requires a small code change. More details to follow
