Change table layout for conditional form

This way the label stays on the left (top) and the forms in the second column.

I rebased this on top of the current master.

I find this gives the edit form a cleaner look.

True. I still thinks it looks nicer / easier to parse. But I agree it's not ideal. Unfortunately, it will be some time before I can investigate a nicer solution.
