CalBug: add "Other Information" field

A lot of people have been wanting to type label information that does not belong in one of the existing fields. We would like to add a field for this called "Other Information." It should appear last in the series of fields.

From Rosie: One thing about the "other field" is that it does allow people to capture the - often very cool and interesting - information on the label. This was something our students brought up (in the days before the "other" field was there): They felt frustrated that they weren't able to do anything at all with the most interesting information on the label. I wonder if the SERNEC people (who are not as much participating on the Github issue ticketing, right?) would be interested in this too? How do we get ideas sort of spread between the projects?

Thanks @robgur ! I would be interested in adding this field to the herbarium interface as well. I think the main reason would be improve the user experience, since I imagine it will be hard to utilize the data that is captured there.

Talked to Chris S. about this at the meeting. He told me it would be easy to add if we expected it to be used. I met with the team here we all agree it will be very useful and should be added before full launch.

For bug data lots of people are putting "other" info in the talk and discussion comments. So this will be good thing to add soon. @denslowm it this also happening with herbarium entries? @chrissnyder
