optimize memory usage
optimize out a couple of loops
minimize in-loop tests
fix possible invalid usage of upper_bound return value
replace 'debug' global variable by a compile-time define (compile using 'scons debug=1' or set _DEBUG define)
replace usage of c++11 lambdas and 'nullptr' for more conventional function objects supported by a wider range of compilers
add a README.md
