• Fixed up some issues with the README
  • broken logo, broken links, inconsistent titles, markdown lint errors, etc
• Added a little more info to the README
  • type definition info, a "whos using common-tags" section, etc
• Extracted out the table of contents generation into its own npm script
• Removed wallaby.js (as I don't use it anymore)
• Generated a package-lock.json for npm v5+
