Added extra columns in d2d html table from data in details JSON column. #199

What'll happen if other states want to use it? Or if we want to add more columns?

I suggest taking a config option DOOR2DOOR_EXTRA_COLUMNS config parameter and use that to add the additional columns instead of hard coding it.
