Move from fees to orders and order items #303

Hi,

This PR is still a draft as it's a work in progress. Currently all Membership stuff is moved over and sorted, and all works with orders, but the eventRegistration stuff does not.

There will be a complimentary PR in Library that is fairly sizeable to back this one up too.

@rugulous please could I have as much feedback as possible as I think I've spent too long staring at it all and have probably done some daft stuff.

@matt-fidd don't suppose you ever got payments working? If so, do you remember how it worked/where the code is?
