Test Form Manager (Forms Controller) #204

Test everything you can in the form manager. Click all the links and test all the features. Please also add QA tests for that in the relevant doc. This is just for the Forms controller (not questions or condition managers). Not much to do here.

If you notice anything unusual, create issues on GitHub and assign me. One issue per problem and add the steps to reproduce the problem.
