I copied the PAQ-C from its data dictionary, which has everything we need (according to the structure that I diagrammed in issue #1), into a single-spreadsheet workbook formatted to import into SQL.

http://mentalhealth.io:5000/ has a template and instructions, and after prepping a questionnaire, you can try converting it on that page; it'll either work or give an error.
