Add clarifications to getReferenceKey #169

Actioning the decision from the meeting on October 3:

WORKING DECISION for spec on GetReferenceKey() Implementations:

MUST support relative references SHOULD output NULL if not able to generate a key MAY support more types of references MAY generate error list of unprocessable references in an implementation specific manner Add high-level guidance on suggestions for contained resource references. Link back to the data layer and FHIR spec sections.
