Major Refactor fixes #46

This PR contains the most recent updates to the environment and documentation.

It also introduces a partially complete feature for collecting all practitioner data from each endpoint, this feature is incomplete

I definitely should have done the fixes in another branch.

This PR should not be merged due to search_all_practitioner_data being incomplete, but due to the need for those fixes I think we should merge.

@kecarman74 Could I maybe get a hand in removing this erroneous merge from the history if that's a thing? I reverted it immediately but that created another PR and just kind of mucked up the history
