Practitioner and Practitioner Role queries #8

This PR brings Practitioner and PractitionerRole searches up to the level they can at this time. These queries are not done, there is still finishing touches to be done.

Next steps are blocked by #6 (Data Standardization) and #7 (Consensus Model - Data Input)

Still need to do Location and Organization Queries
