Oesophagitis and Oesophageal Ulcer #116

@finersarah I think this list requires discussion: 1) There are many codes for reflux oesophagitis, but also infectious oesphagitis (phlegmonous, gangrenous, fungal ulcer) and chemical. This may be appropriate, or we may want to seperate out the reflux associated oesophagitis. 2) If we are including infectious causes we should extract codes for oesophageal candidiasis (as this results in an oesophagitis). - I will list these codes in a comment below. 3) At the last meeting we decided to avoid combining the establishde codelist phenotypes at this stage. So I have not combined codelists for the different reflux phenotypes (e.g GORD and oesophagitis). However if we do decide to do this later we can combine the reflux oesophagitis with GORD and have a separate list for non-reflux oesophagitis.

@f-eto Codes for oesophageal candidiasis pending discussion. There will be overlap between this list and the HIV list.

(FYI @f-eto oesophageal candidiasis is a fungal infection of the oesophagus that causes inflammation, it is associated with immunodeficiency as seen in HIV)

I would leave this as a broad category for now, and include oesophageal candidiasis using the codes above. It makes sense that Barrett's oesophagus has a separate codelist as this is a pre-cancerous condition and therefore will be associated with different monitoring, treatment and outcome.

NB this codelist is very closely linked to gastro-oesophageal reflux disease (GORD), although the pathologies have some distinction - GORD commonly, but does not always, cause to oesophagitis/oesophageal ulcer. There are wider, non-GORD, pathologies that cause oesophagitis/oesphagitis.
