Basically, instead of just the typical, get shot and lose health and then regenerating that health, I had something a bit different in mind, introducing status effects and crippling.
Crippled limbs (head,torso,legs,arms):
Same mechanics as in NV, if your limbs receive a certain amount of damage, whether it be from bullets or potentially fall damage if that's ever added, it will be crippled, you can see your limbs and their health in your pipboy, they can be healed with healing items that will be added in the future, depending on the body part, if your crippled, it will affect your accuracy and movement speed, or blur your vision, same stuff as NV essentially.
Regeneration:
You only regenerate a quarter of the amount of health you lost, if you aren't hungry, to actually regenerate or insta heal you will need stuff like stimpacks, etc.
Afflictions:
Pain (minor, heavy, severe):
If you lose a certain amount of your health, you will experience the affliction, pain now depending on the severity will be on the amount of health you lost, so if you take damage normally without armor, you will experience pain faster, but with armor, you will need to take more damage to experience pain obviously. Pain can be treated with med-x or painkillers, something like that and will give pain resistance for like 5 minutes, or you can wait a certain period of time for pain to go away. If you are inflicted with pain you will have a blood covered screen, that will be more bloodier depending on the severity, you will also be less accurate when suffering heavy pain and slowed down in walk speed when suffering severe pain.
Radiation:
You will have a geiger counter that will go off in your pipboy that will start to crackle when you are inside an area that has radiation, you will also have a counter of how much radiation you have been exposed to. If you reached a certain amount of radiation you will get, radiation poisoning. If you get even more radiation after getting radiation poisoning, you will die, if suffering from radiation poisoning, you will have a reduced max amount of health, health regeneration is slowed, and healing items aren't as effective. Treating radiation and radiation poisoning requires rad away, using rad x temporarily removes radiation poisoning if you have it and gives resistance to radiation for a certain amount of time, once that timer runs out, you will get the affliction again if you had it, rad away will simply make you hungry and thirsty and slowly drain a certain amount of radiation per use.
Bleeding (minor, heavy):
Bleeding can only be caused by taking damage from weapons, if struck by a weapon, there's a chance you will receive the affliction, when you receive the affliction it will always start off as minor, you will see some blood particles coming out of your character, bleeding can be stopped with bandages, if not treated after a certain amount of time, it will become heavy bleeding, it will slowly drain your health, if not treated, you will bleed to death, if treated, you will have the blood loss affliction, which can be treated with a blood bag, if not treated, you will not be able to regenerate health, it can also be treated slowly by eating food.
Notifications:
Obviously get notified when you get an affliction.
Be able to read about the affliction:
Hovering over afflictions will show you the treatments and a short description about it.
I'm not saying you do everything this suggestion says, just take bits and bobs from it, i'm just giving out ideas.
