Add Operational Analysis Endpoint

IT Collect is adding an Operational Analysis endpoint to collect the data found in Operational Analysis Table D.1 of the CPIC policy guidance. The Operational Analysis collection will contain the following fields:

• Date of Analysis
• Analysis Results
• Analysis Conclusion
