Prepare for v4 #548

This PR will bump all dependencies to their latest versions, in preparation of releasing v4 of this library.

I'm having some issues with stalled builds in Travis for Node 12 and 13. I can't repro this locally so I'm testing out things in a few WIP commits.

I haven't figured out how to resolve the codeclimate issues (they weren't introduced by this PR). I'll go ahead and merge anyway. @lencioni what are the actions I need to take?
