miscellaneous field duplicated #24

there's two miscellaneous fields in the Common Metadata Form. One is free-text, one is boxes.

The intent was the free-text field is things that can not be mapped as a dictionary of key-value pairs. But i see the confusion. We should probably only keep one. Any preference?

The miscellaneous free-text that is not hashable seems redundant with sample description, so maybe leave only the hash?
