Treat the supervisor code as a python package. Changes include:

Updated the imports, and removed pythonpath hack.
Added license file (MIT)
changed configs to be in instance/config in accordance to flask best practices.
Startup command is now python -m host_app
Line length is now 120 characters.
Use the /app folder for vscode workspace.
