add MPI_Type_create_hvector #46

Needed for newer versions of PIO.

Can you make this PR to https://github.com/MCSclimate/mpi-serial ? mpi-serial is now a separate repo brought in as a subtree. I'll include this in the upcoming MCT 2.10 release.
