Yeah. I didn't go through all the interation of NPC entities to see if it does the same with riot guard for example. I'll test with those later.

No. I didn't go through them all yet.
I went and changed to the RIOT_NPC_SHOTGUN character and the same issue shows up only he's moved from his spawn so the shotgun is already spawned in place when I view him. And his weapon still vanishes when he's killed.
Seems like it's across all characters.
Also, not sure it's vald or not. I spawned a heavy joe near the who characters from earlier and they were both missing when I got into the mission, but there were two shotguns and two pistols floating instead of one each along with the heavy joe. There were no bodies. I'd need to make sure on this part that is was not just an issue from the save file. I've been cloud saving these files since the last "can't save" bug. Steam sometimes restores things when I don't necessarily want it to though.
I started with a fresh commands.pak and tried experimenting with character spawn again. Spawned a shotgun character from \human\security\grunt_npc_shotgun . Set position near start of M3. When the character spawns with their shot gun and moves the first time the shotgun separates and stays floating in air. Upon killing them the weapon they're holding vanishes but the weapon in air remains floating. I did same thing with \human\security\grunt_npc_pistol and spawned them near the first. The pistol does same thing it separates from where they were spawned and remains in air.
I made two video clips. They are not previewing correctly in github preview mode.
IDK if they're working right or just need to be processed before they work.
