Add ability to add multiple filters using the shortcode builder #12

Have a add filter to shortcode button. Once added have an x button to remove it from the shortcode or a click to edit it.

Can't have completely separate filters, e.g. [ghthumb filter="blah" filter="boo"]. Will need to implement multiple filters using logic.
