Encourage the user to enter a short description of what they were doing when they crashed #26

While a user can technically do so at current by editing it into one of the log tabs we should encourage it with a separate field. Depending on the upload service it could go in its own section (Gist) or just get be pasted at the top of a multi-item pastebin entry.

It may also help to put a disclaimer somewhere that simply uploading a paste doesn't report it to us, or at least it didn't before I enabled alerts on pastebin a few days ago. I'm getting paste alerts without matching user reports anywhere. Most of them are either free of errors, have classic ones like out of memory or graphics device not supported, but occasionally there's an interesting one like http://pastebin.com/XNQMvaJX which seems to indicate a shader didn't work right - usually that's probably still old video card / bad driver but it may be a real issue that's just ignored or hidden on most GPUs.
