Extract Questions app allow the user to select an EQ Survey JSON file and extract a table containing the questions, answers and options in CSV format.

The initial page allows user to select JSON file. The results form displays a table with the extracted questions and give option to download a csv file of the data or to return to the previous form to select a new JSON file to analyse.
