add user survey to top of website #63

Description

I have seen it in various websites. Right now, it states "Welcome to the New OPeNDAP website!". I propose we add somerthing ike:

Thank you for visiting - please consider filling out this User Survey to better understand your needs

We can then ask questions like:

  1. Where are you from
  2. Professions (pick from a list of some professions - add other)
  3. Highest education level
  4. Interested in accessing data or serving data
  5. Gender and Ethnicity
  6. Access pattern (matlab, python, browser, other)
  7. How did you find out about us (google search, redirected from other website)
  8. [optional] What is a feature that you are most interested?

I have:

a) Created a User Survey b) Added a link to the header that shows in all pages of the website c) Created a button on the Home page, the most visited page of our website, so increase visibility to the survey.
