new enhanced_ref_data missing required columns #55

The new enhanced REF database ./data/final/enhanced_ref_data.zip is missing the following required columns from the previous version:

ics_id
countries_iso3

We need to add these back in to the final data.

ics_id is the same as\superceded by REF impact case study identifier (i.e. a UID used throughout the make_enhanced_data.py script.
countries_iso3 has been superceded by countries_extracted, see make_countries_file().
