For the sponsor to be able to setup trial specific questions, the sponsor app should allow creation of QuestionTypes

Pre-release v0.8.0 shows experimental feature:

During the editing of trial-specific questions, it is possible to create new question types (and then define an eligibility criteria on them). Experimental (there are no features to delete new question types, and neither to look for similar existing question types).

When adding new trial-specific questions, these should be added to the eligibility criteria by default. It seems to make no sense to have trial-specific questions that are not part of the eligibility criteria.

jpsl: Ok to re-open, but leaving that for after september...
