Form definitions in ctr-patient/ctr-dsu-wizard/formDefs/index.js and other formDefs returned by services such as /borest/ctrms/trialPrefs use hardcoded URLs like https://ctr2-dev.pharmaledger.pdmfc.com/borest/lforms/medicalconditions on a different web domain to workaround PWA restrictions in accessing nework.

Desired fixes/options:

• do not use hardcoded URLs so that each environment uses a relative path ?
• (pre)transform the URLs for each environment ?
• make the PWA allow/unblock these URLs on the same domain ?

If commit 790598e works OK in DEV and TST, then this issue can be pushed to "ToDoAfterMVP", and give us some time to research where the PWA network is configured.

@VNogueira91 reported that the ctr-patient/README.md instructions to use the ctr-dev REST services are not working.
