patient-ssapp v0.11.5 take it out of demo mode

The patient-ssapp is in "demonstration mode" with pre-filled credentials (and use pre-created by the test script ests/ctr-dsu-wizard/ctr-dsu-wizard-test.js ), so that anyone can just click the SIGN IN button, and login.

For the pilot, it is desirable that:

• on first use, patients are taken to the SIGN UP page (not the SIGN IN page).
• on subsequent use, patients are taken to the SIGN IN page (with prefilled credentials that they used on the SIGN UP page, if successful).
• the SIGN IN page should not display the "Repeat your Password:" field.
