Use common protocol.h for mvdsv. #113

Hey nano,

Thanks for this PR. Especially the CMake based submodule pull/update is sweet! We gonna use it in the other repos for sure!

There are some defines hardcoded now for mvdsv to make it build with the shared protocol.h, so those SSWeapon ifdefs are won't needed... So at the end I think I won't merge this, but using your code in #114 .

Also, what is that gen-release thingy?

gen-history was requested by VVD. Regular source downloads from github doesn't include submodules, so automatic source downloads will not build. Matter of taste if you want to start releasing source tarballs with releases. Linux/BSD distros usually grab the release tarball and build from that, so that is what prompted this.
