Various minor link fixes #42

Inspired by QuiltMC/quiltmc.org#24

Capitalise Discord in links to the Discord server
Fix RFC6 links in RFC7
Add all relevant repository links to technical teams
- Replaced yarn with quilt-mappings
- Removed intermediary, matcher, and matcher-mc
- Renamed ASMR to CHASM and added chasm to that team
- Added subteams (grabbed from the roles list in toolchain) and fabric-api-qsl to QSL
- Added launchermeta-parser to infra
- Added mappings-hasher, quilt-mappings-on-loom, unpick, and javadoc-draftsman to mappings
- The diff for this part is kind of useless, so here's what it ends up rendering as: Rendered view

Unresolved questions:

Why do the yarn, intermediary, matcher, matcher-mc, and fabric-filament repositories still exist?
Do we need to list every single repository a team is responsible for in this document? Will this get tedious after a while, or will it be easier to maintain over time as less new projects are created?

Part of these have already been fixed by the other recent PRs, so I'll be closing this. It's worth noting that the teams still needs to be updated, especially with the new Community Tooling team.
