Add ability to generate a new record for any configured table.

Need to have a .vscode folder for each instance where we store the record template...

Will be a JSON payload of the record to be created.
JSON Payload should store field label, name, and value (we will build into correct payload structure)
- Also an attribute for "PromptForValue" true/false that we will use in the creation wizard.
For any blank values in the template, prompt the user to fill them in
- This will be used for things like "Name" of a script include etc.

Flow for new record.

Prompt for instance (if multiple)
Prompt for App scope (if multiple)
Prompt for Record Type
For each attribute with "PromptForValue:True" in our template, present input box:
- The label (column_name) [app_scope] value?
