[UI] Create intial components for search of appropriate clinic\doctor #38

AC1: create UI component to select unwellness case and other criteria of doctor's search. Use initial structure like on wiremock below. Optional: component might have integration with survey form to help user with identification of his case.

AC2: create UI component with list of ordered recommended doctors for the given case. Use initial structure like on wiremock below. For the first version to show all doctors from db. Each record shall have link to component to pickup appointment
