Add environment checkboxes to issue template #109

Add list of checkboxes to Your environment section of the ISSUE_TEMPLATE. This will make it easier for new issues just to tick the relevant boxes when they submit a new issue.

Yeah, I have seen this type of approach in some other projects and it seams to working.
