I had planned to use this issue to collect comments/questions, but just found it in my collection of open tabs with this one bullet point. Maybe I'll file other issues when I get back to this later?

• What's going on in Eq (3)? Filling in an intermediate line, we have $\lim{h \searrow 0}\frac{1}{h} \int [(y - x){+} - (y - x - h)_{+}] dF(y)$. Now, if y > x then this works out to (1/h) * [(y - x) - (y - x - h)] = h/h = 1?

Thank you!! The signs were screwed up on the first lines of both derivatives. I am changing $(Y-x)_{+}-(Y-x-h)_{+}$ to $(Y-x)_{+}-(Y-(x-h))_{+}$ for $D{-}$ and $(Y - x + h)_{+} - (Y - x)_{+}$ to $(Y - (x + h))_{+} - (Y - x)_{+}$ for $D+$...

Will push shortly.
