Bug when updating duty step #382

To recreate this bug please do the following:

  1. Click on this link: http://dev.melbourne.wcewm.co.nz/Duty/admin?action_id=20&task_id=12
  2. Next, click on the update button next to the top duty step "Issue outage info..."
  3. Goes to CDbException error

Also, secondary issue is that the duty steps all come up with a pencil. Not sure this should be like this.
