Error when trying to add new Action to a task #390

To recreate this bug, please do the following:

  1. Click on this link: http://dev.melbourne.wcewm.co.nz/TaskToAction/admin?task_id=12
  2. Click on +New button in Actions screen
  3. Select LiveLine Application (SP AusNet)
  4. Now says "Duplicates not allowed", however as you can see it is not showing in admin view. Also, first time I tried to add it, it threw an Exception error.

@hawea please confirm resolved and close - next upload shortly. Scoping error on mode, i.e. shouldn't have been allowing selection of line line application when in outage mode
