Include Dependency Query Parameters #65

On the page that Apex Designer uses to create an app, add is query parameter that instructs the page to make an optional App parameter required to ensure that the dependency is added.

The App Manager template will have optional App parameters for Postgres and Camunda 8. If the user checks the box to include those, the query parameters will be added to ensure that the dependencies are created (if none exist) and added.
