Add support for multiple GoCardless accounts #47

The requirement is to be able to setup multiple payment processors that use different GoCardless accounts, e.g. one for a charity and one for a company.

Currently the extension assumes there is only one GoCardless payment processor.

There's a beta of this feature in branch https://github.com/artfulrobot/uk.artfulrobot.civicrm.gocardless/tree/issue/47 if anyone wants to help test.
