BCPL Locations Page: Incorrect contact form loading for Arbutus #638

Another fun issue for the BCPL locations page: https://www.bcpl.info/locations/index.html

It looks like the Arbutus branch is loading the wrong contact form. The heading for the form is correct, and the ID in the JSON is correct, but the wrong form is loading. Seems to only affect the Arbutus branch.

Released to prod. Will close pending full testing by @jdomasky. Jason, no rush -- let me know when you have availability.

The BCPL Locations Page in prod passed the Locations Page Test Plan. The issue with the Arbutus contact form seems to be resolved.
